Subject         :           UPDATED GUIDELINES FOR MAKATIZEN ECONOMIC RELIEF

                                    PROGRAM

Dear Unit Owners/Residents,

Please be informed that Makati City is giving Php 5,000 economic relief to all qualified Makati Citizens affected by COVID-19 pandemic.

Kindly note the following guidelines:

Qualifications (ALL should be met to be qualified):

1. All Makatizens 18 years old and above who lives in Makati City or those who were relocated in the areas of Laguna, San Jose del Monte, Bulacan.
2. Registered Makati City voters before May 13, 2019.
3. Registered under any one of the following Makati Cards:

• Makatizen Cardholder
• Yellow Cardholder under Makati Health Plus Program
• Voter’s Certificate or Voter’s ID

Requirements:

1. Completely filled out Makatizen Card Application Form (hardcopies available at your respective Concierge)
2. Photocopy of a valid government issued I.D.

Claiming of Economic relief:

1. Makatizen e-wallet account for existing and activated Makatizen Cards
2. GCash mobile account number nominated in the application forms for new applications

Instructions and Reminders:

1. Kindly submit all forms and requirements preferably in a sealed envelope to your respective Concierge. Please be informed that Makati City has set no deadline yet for the submission of the said forms and requirements. Building admin will request Barangay personnel to pick up the application forms submitted at the Concierge on a regular basis.
2. Please be guided that per Makatizen Economic relief guideline, monetary assistance must be used within 30 days (fully or partially) upon receipt to prevent being invalidated.
